Before you can start correcting crowding, spacing, or bite issues with clear aligners, your mouth needs to be a stable, healthy foundation. If you have an untreated cavity, a tooth that needs a root canal, or gum disease simmering below the surface, moving teeth around that instability can create bigger problems than the ones you started with.

At A1 Dental, we take a sequenced approach to treatment planning rather than jumping straight into aligners. As a general dentist who also offers clear aligner treatment, we look at the full picture of your oral health first, addressing decay, infection, and gum concerns so the foundation is solid before any tooth movement begins.

Why a Full Exam Comes First

Every orthodontic case at our office starts with a comprehensive exam, including digital X-rays that let us see below the gumline and between teeth where problems often hide. We are checking for active decay, bone levels, and any signs of infection around the roots. Straightening a tooth that has an undiagnosed problem underneath it can mask symptoms or make an existing issue harder to treat later.

This step matters more than it might seem. The American Association of Orthodontists notes that a thorough in-person exam, including x-rays and a review of past dental records, should always happen before any orthodontic treatment begins, since it gives your dentist the full picture needed to plan safely. Addressing decay before treatment begins, rather than during, gives you a much better starting point.

Treating Cavities and Infections Before You Begin

If your exam turns up a cavity, we address it before your aligner treatment starts. We rely on minimally invasive options like drill-free cavity treatment whenever a cavity is caught early enough, which allows us to stop decay without removing healthy tooth structure. For deeper decay, restorative work like tooth-colored fillings needs to happen first, since a filling placed after aligners have shifted your bite can end up sitting incorrectly.

If a tooth is infected all the way to the nerve, it needs a root canal before aligner treatment can move forward. Moving a tooth with an active infection underneath it can allow that infection to spread, and it can also make the tooth more sensitive to the pressure aligners place on it. Getting this resolved first protects both your comfort and the long-term health of that tooth.

Extractions are sometimes part of the plan too. When a tooth cannot be saved or needs to come out to create space for alignment, we use piezo surgery, an ultrasonic technique that preserves the surrounding bone during removal. Keeping that bone intact matters for your long-term oral health, whether or not you eventually need an implant in that spot.

Gum Health Cannot Be Skipped

Moving teeth puts pressure on the gum and bone supporting them. If gum disease is present and untreated, that pressure can accelerate bone loss around the affected teeth. We check for early signs of gum disease at your exam, and if we find them, treatment happens before aligners, not alongside them.

Depending on what we find, this might mean a deeper cleaning, or it might involve technology like laser therapy to address inflammation more precisely. Some patients benefit from airflow polishing, which uses a fine, gentle powder to clean the tooth surface smoothly without irritating already sensitive gums. Whatever the right approach is for your case, getting your gums healthy first gives your teeth a stable foundation to move into.
